About Us:

The Cottage Bridge Foundation is a 501(c)(3) nonprofit organization governed by a volunteer board of directors. The Cottage Bridge Foundation is a community for adults with Intellectual Disabilities, Alzheimer's, Dementia, and acquired brain injuries, nestled in Snellville, Georgia. The mission of The Cottage Bridge Foundation is to provide progressive life assistance to its special population so that they can maximize their abilities and maintain their independence in the least restrictive environment. The Cottage Bridge Foundation offers Respite Care, Hospice Care and non-residential programs that offers Adult Day Health services. The Adult Day Health Service offers a balance of structure, freedom, encouragement, instruction and fulfillment. In addition to skilled nursing care. The Cottage Bridge Foundation also offers on-campus health services, educational programming, some clinical trails ,vocational training and job placement, and a vast array of community inclusion activities. The Cottage Bridge Foundation Respite care also allows the patients to be admitted into our facility to a inpatient bed for 1-5 days to provide a break for the family or caregiver.

Founded August 21, 2019 by Alejandra K. Lackhan, a dedicated wife and mother who was raised with a brother that has special needs, who wanted her community to have an opportunity to give back to those more in need such as special needs, elderly and dementia participants who are not able to do for themselves. The Cottage Bridge Foundation became a place where adults with intellectual disabilities could call home.

Thanks to continuous growth, The Cottage Bridge Foundation, many significant milestones have been achieved in serving individuals with Intellectual Disabilities, Alzheimer's, Dementia and acquired brain injuries. With a host of volunteers, The Cottage Bridge Foundation provides a comprehensive range of programs designed to improve the quality of life of each individual served, according to his or her own unique needs and abilities.

The Cottage Bridge Foundation has remained committed to promoting the personal growth and self-sufficiency of adults with Intellectual Disabilities, Alzheimer's, Dementia and acquired brain injuries - a truly undeserved population.
